The easiest place to do this one is 8-3. Time it just right so that you fall down a hole just above a Hammer Bros. hammer. The hammer will fall on you and you will die twice if you do it just right.

In 8-4, if there are Piranha Plants in the pipes you're supposed to go down, they disappear when you get too close to them.
